Aimed at decision makers, intelligence leads, digital leads, clinical leads, and anyone involved in population health intelligence. The day's agenda will feature:
- Exemplar population health intelligence solutions and approaches in the North and beyond – an opportunity to hear from those who are leading the way in initiatives to understand population health need, identify issues and opportunities for improvement, target patients who have rising risk levels or are impactible, and predict and evaluate impact
- Workshops to increase understanding of the capabilities needed to create robust PHI; and to increase high level understanding of the types of analysis that can be used to inform quintuple aim improvements (better health, better experience, lower cost, improved staff work life and equity)
- A preview of the National Performance and Population Health Management Dashboard
- An opportunity to link with like-minded people to enable collaborative approach to dealing with our highest healthcare priorities
