Our Communities of Improvement (CoI’s) have been established to foster a joint approach for the development of an outcome based work plan that will support the delivery of public health outcomes with Y&H. Each Community of Improvement is jointly supported by the YHADPH and OHID to establish a collaborative approach for the development of an outcome based work plan that will support the delivery of public health outcomes with Y&H.
