Privacy Notice

Public Health Practitioner Programme Team Activities

This privacy notice applies to you if you are:

  • A practitioner/assessor/mentor/verifier for the UKPHR Y&H Scheme (Portfolio Route).
  • An applicant for a place on the UKPHR Y&H scheme.
  • A manager of an applicant for a place on the UKPHR Y&H scheme.
  • A participant of any of the training or other events organised by the Public Health Practitioner Programme Team
  • Someone who has expressed an interest or is participating in programmes/activities/events organised by the Public Health Practitioner Programme Team

Information we collect

We collect information through on-line registration forms, application forms to join our scheme as a Practitioner, Mentor, Assessor or Verifier, when you contact us to express an interest in our services or when you sign up to our mailing list.

The usual categories of personal data we will process about you include (but are not limited to):

  • The contact details that you provide to us, including names, titles, addresses, telephone numbers and personal email addresses.
  • Personal details/data such as date of birth, gender, etc.
  • Your position, role, employer and skills for health level.
  • Training records.
  • Records about your application and recruitment to the scheme, including your application paperwork, details of your qualifications/education, references, requests for special arrangements, communications regarding our decisions.
  • Details of any medical issues and/or disabilities that you have notified to us, including any consideration and decision on reasonable adjustments made as a result.
  • Learning and development records, including your attendance, completions, accreditations and certifications.
  • Computing and email information, including login information for the Programme’s IT systems.

How we use your information and our lawful basis

We administer, monitor attendance and evaluate your participation in activities for the Public Health Practitioner Workforce Development Programme, Core Programme, UKPHR Yorkshire and Humber Scheme and associated events as a task in the public interest.

Specifically, we use:

  • Contact details to keep in touch about our activities
  • A log of attendance at training and events to understand which roles are taking part
  • Expressions of interest in activities to understand how we can improve them
  • Your opinions to evaluate and improve activities
  • Anonymised quotes to promote our activities

Marketing and promotion

If you give us your consent, we may contact you to:

  • Send you information about our events and initiatives.
  • Invite you to become a practitioner, assessor, mentor or verifier.

You can unsubscribe or update your preferences at any time by emailing PHPPYH@leeds.ac.uk

We may record our events to make them accessible for all, promote our activities or to report on the work that we do as a legitimate interest of the organisation.  We will always let you know in advance.

Special Category Data

To make our service accessible we may ask you for your health, dietary or religious requirements, we will only use this information if you give us your consent.

How long we keep your information

We keep your personal information for as long as is necessary to fulfil the purposes we collected it for.

We may anonymise your personal information so that it can no longer be associated with you, in which case we may use such information without further notice to you.

Who might we share your information with?

We may share your data with other organisations who assist us in providing the activities you take part in; we have agreements in place to protect your . These organisations include:

  • UKPHR
  • Leadership Centre
  • Health Talks
  • NHSE
  • Stirling Potential Ltd
  • Humber Generalist School (HGS)

When joining the UKPHR Y&H Practitioner Registration Portfolio Route (scheme) you will be asked to join Khub and will be registered on E-portfolio.  To attend our core sessions you will be asked to register with Maxcourse. If you choose to attend a masterclass session by the Faculty of Public Health you will be asked to register with them to receive attendance information.

We may also include statistical information (this is anonymised) in internal and external reports and articles, both physical and online. Examples would include reports to the Steering Group, NHSE, UKPHR other partners we work with to deliver and evaluate the scheme.  

How do we keep this information secure?

Your data is stored in secure systems in line with the University of Leeds Information Protection Policy as follows: SharePoint folders, University network drive, Microsoft Forms, Jisc Online Surveys . 

We limit access to your personal information to those who have a business requirement to know. They will only process your personal information on our instructions, and they are subject to a duty of confidentiality.

Your data will not be transferred outside the EEA/UK. 

How can I have my data removed or deleted?

We need to process contact details for you so that you can take part in our programmes.  If you do not want us to use this data in this way, then unfortunately you will not be able to participate.    

When applying for the portfolio route we need to check you are eligible to be offered a place. If you do not want us to use your data in this way then unfortunately, we will not be able to check your eligibility and you will not be able to continue in the process.

To make activities inclusive of your needs, we ask you to tell us about additional needs you may have. We will only do this if you give us your consent, but if you choose not to we may not be able to support you fully.  

Please note that once your data has been anonymised for statistical purposes it will no longer be able to be identified for deletion.   

Your data protection rights

You have the right to: 

  • withdraw consent at any time where that is the lawful basis of our processing; 
  • access your personal data that we process; 
  • rectify inaccuracies in personal data that we hold about you; 
  • be forgotten, that is your details to be removed from systems that we use to process your personal data; 
  • restrict the processing in certain ways; 
  • obtain a copy of your data in a commonly used electronic form; 
  • object to certain processing of your personal data by us.
